Do you accept insurance for physical therapy services?

No, we do not accept insurance. To offer you the best, most personalized care, we have chosen not to deal directly with insurance companies. There is an option, however, for you to request a detailed receipt called a superbill from your visit(s) at Elevate which you can submit to your insurance company for out-of-network physical therapy. Your insurance provider would then reimburse you directly based on your out-of-network coverage. We recommend calling your insurance company prior to initiating therapy at Elevate Health to gain understanding of what your out-of-network physical therapy benefits are. There is an insurance worksheet that we can provide you so that you know which questions to ask your insurance provider. If you decide that you’d like to submit to your insurance, simply request that we provide you with this superbill at or before your visit.

Do you accept insurance for health  & wellness coaching services?

No, we do not accept insurance for health & wellness coaching services. See the answer to question #1 above for more details. At this time, unfortunately there are very few insurance companies that are even reimbursing for health coaching services (despite it’s proven efficacy), but you may wish to call your insurance company to see if health coaching services are covered. If so, it would be up to you to request a superbill before or at each visit that you could submit for reimbursement on your own.

Why don’t you accept insurance for your services?

There are a number of reasons that we have chosen NOT to accept insurance. The landscape of our healthcare system is rapidly changing and insurers are demanding more and reimbursing less for physical therapy. It allows us to keep our rates lower than what typical charges would be for 60 minutes of physical therapy billed through insurance. This matters if you have a high deductible (which many plans do these days). We will expect to work together once per week, which is less frequently than the 2-3 times per week at a traditional clinic. This matters if you have a high co-pay. Our hour-long sessions will be one-on-one and personalized to your needs. Instead of having to spend after hours dealing with insurance companies, we are able to put this time toward more client visits, planning your care & learning more tools to assist in your healing/elevating your health. Finally, insurance companies often dictate the care that is provided. By not accepting insurance, we can structure your care based on what you need rather than what an insurance company requires.

I have Medicare. Can I be seen at Elevate Health?

It depends. Unfortunately, Medicare does not allow it’s enrollees to receive physical therapy services from an out-of-network physical therapist. This means that, if you are needing care for a specific problem that is affecting your daily function (such as low back pain, frequent falling, shoulder weakness, etc.), you are unable to come to Elevate Health for your care. HOWEVER, there are certain things that Medicare will not pay for in a clinic that accepts insurance that you can then use your own funds to pay for. Any service that is for wellness or prevention (such as health & wellness coaching) is not covered under Medicare and thus can be purchased from a qualified provider such as Elevate Health. It is our hope that these rules may change someday, but until that time, if you are needing physical therapy for a specific problem, I am happy to recommend a clinic in the area that does accept Medicare.

Can I submit my physical therapy or health coaching receipt to my HSA or insurance company on my own?

Yes, you can. We are able to provide you with a detailed receipt called a superbill from your visit(s) which you can submit to your insurance company for out-of-network physical therapy.
